Before filing a compliant, you may want to resolve any differences by first contacting the Responsible Funeral Director at the funeral establishment.  You may also contact the Funeral Board directly to seek assistance at 602-542-3095.  Persons who wish to file a complaint against an establishment, funeral director, embalmer or other persons regulated by the Funeral Board should complete the online complaint form below, including supporting documentation.  Please make sure to include the name of the decedent and the date of death in your documentation.
