Arizona Department of Health Services – State Bureau of Vital Records, Database Application for Vital Events, or “D.A.V.E”.

As most of you are aware, the State Bureau of Vital Records has acquired a new database application of vital events, "D.A.V.E.".  This system will make Arizona 100% electronic regarding death registration.  With D.A.V.E., medical certifiers will be given an identifying password.   All physicians will be required to certify death certificates electronically within system.  In Arizona, complete registration of the record must take place before a funeral home will be able to print a disposition transit permit.

The Governor's Office has begun a rollback initiative and are currently seeking input from the regulated community.
