Board meetings will begin promptly at 9:00 am at 1400 W. Washington Phoenix AZ in Conference room B-1. Additionally, the Continuing Education Advisory Committee meets quarterly (January, April, July & October) at 11:00 am or following the completion of the monthly Board meeting (also in Conference room B-1).

Arizona Department of Health Services – State Bureau of Vital Records, Database Application for Vital Events, or “D.A.V.E”.
As most you are aware the State Bureau of Vital Records has acquired a new application of vital events. The anticipation is that this will make Arizona a 100% electronic state. Something that is changing is that with D.A.V.E. medical certifies will be added in this electronic system in addition to Funeral homes and others who already have access to VSIMS. So these medical certifiers will be given an I.D. and password. All physicians will be required to certify death certificates electronically with this new system. Arizona has adopted a work flow that requires complete registration of the record before a funeral home will be able to print a disposition transit permit.
